During Episode 146, Ms Digby addressed whether or not a fall can be reported on the UB-04 or the IRF PAI if it happened on the day of discharge from an IRF. Our friend, Anonymous, asked also if the physician documentation states that the patient has DM (diabetes mellitus) and also states “polyneuropathy of unknown cause” is it appropriate to report DM with neuropathy? The final questioner notes that PT, OT, and SLP and resident (physician in training) notes may contain information that is not validated by the attending physician. Can any of that information be reported?
