Paula Digby speaks to the Insider’s about the “problem with the problem list” and asks for input on strategies that work in your organization. Today’s questions are all from the IRF setting. First, Peggy asks about diagnoses listed by the physician that the coder doesn’t utilize on the UB-04; can they be used on the IRF PAI? Is this a risk? What if that diagnosis is a tier? The second question (from our friend, Anonymous) is like it. Anonymous states the patient is no longer receiving antibiotics but is still on contact isolation for CRE (carbapenem-resistant Entero bacteri acee). Can the klebseilla UTI be reported? Finally, another Anonymous friend asks, with stroke, for neuro neglect, we use R41.4 on the IRF PAI. Does it also belong on the UB-04, or should I69.312 (visuospatial deficit and spatial neglect following cerebral infarction) be used?
