2025 ICD-10-CM (CMS)

2025 ICD-10-CM files

These files include updates for FY 2025. Use the 2025 POA Exempt Codes file and the 2025 Conversion Table for discharges occurring from October 1, 2024 – September 30, 2025, and for patient encounters occurring from October 1, 2024 – September 30, 2025.  Use the 2025 Code Descriptions in Tabular Order, 2025 Addendum, 2025 Code Tables, Tabular and Index, and the FY 2025 ICD-10-CM Guidelines files for discharges occurring from October 1, 2024 – March 31, 2025, and for patient encounters occurring from October 1, 2024 – March 31, 2025.

These files include updates for FY 2025. Use these code files for discharges occurring from April 1, 2025 – September 30, 2025, and for patient encounters occurring from April 1, 2025 – September 30, 2025.

Note: There are no changes to the 2025 POA Exempt Codes file or the 2025 Conversion Table.

2024 ICD-10-CM (CMS)

2024 ICD-10-CM files

These files include updates for FY 2024. Use these codes for discharges occurring from October 1, 2023 – September 30, 2024 and for patient encounters occurring from October 1, 2023 – September 30, 2024.

The ICD-10-CM April 1, 2024 update addresses typographical errors. There are no new diagnosis codes being implemented. The files below contain information on the ICD-10-CM updates effective with discharges on and after April 1, 2024.

2024 ICD-10-PCS files

These files include updates for FY 2024. Use these codes for discharges occurring from October 1, 2023 – September 30, 2024.

The updated ICD-10-PCS code files include 41 new procedure codes effective April 01, 2024.

2022 ICD-10-CM (CMS)

On October 1, 2015, the ICD-9 code sets used to report medical diagnoses and inpatient procedures were replaced by ICD-10 code sets. The transition to ICD-10 is required for everyone covered by the Health Insurance Portability Accountability Act (HIPAA). Please note, the change to ICD-10 does not affect CPT coding for outpatient procedures and physician services.

2021 ICD-10-CM (CMS)

The 2021 ICD-10-CM files below contain information on the ICD-10-CM updates for FY 2021. These 2021 ICD-10-CM codes are to be used for discharges occurring from October 1, 2020 through September 30, 2021 and for patient encounters occurring from October 1, 2020 through September 30, 2021.

Note: There is no FY 2021 GEMs file. As stated in the FY 2016 IPPS/LTCH PPS final rule (80 FR 49388), the GEMs have been updated on an annual basis as part of the ICD-10 Coordination and Maintenance Committee meetings process and will continue to be updated for approximately 3 years after ICD-10 is implemented.

We made the GEMs files available for FY 2016, FY 2017 and FY 2018.

An announcement was also made at the September 2017 ICD-10 Coordination and Maintenance Committee meeting that FY 2018 would be the last GEMs file update.

2021 release of ICD-10-CM (CDC)

Note: This replaces the FY 2019 release. These files listed below represent the FY 2020 ICD-10-CM. The FY 2020 ICD-10-CM is available in both PDF (Adobe) and XML file formats. Most files are provided in compressed zip format for ease in downloading. These files have been created by the National Center for Health Statistics (NCHS), under authorization by the World Health Organization. Any questions regarding typographical or other errors noted on this release may be reported to nchsicd10cm@cdc.gov .
