First, Paula Digby shares with the “Insiders” about coders being detail-oriented, including a “story from the road” that makes her point about facilities (and even coders) working in “silos”. Then she weighs in on a scenario described by Donna, from an acute facility: the diagnosis was documented by the physician and treated, but they were still denied by the payor.
