MD documentation should address:

 

  1. Document the condition(s) that warranted the major joint replacement or reattachment of the lower extremity.
  2. Document the following procedure(s) clearly and precisely in the body of the operative report when performed:
    • Resurfacing arthroplasty of total or partial hip
    • Total or partial hip replacement
    • Total knee replacement
    • Total ankle replacement
    • Foot reattachment
    • Lower leg or ankle reattachment
    • Thigh reattachment
  3. Document acute and chronic condition(s) that are being actively treating and/or are affecting patient care. Some of these conditions are considered MCCs, which would optimize payment based on MS-DRGs. Common examples of MCCs include:
    • Septicemia
    • Pneumonia
    • Symptomatic HIV disease
